During a normal menstrual cycle in a woman, there is an interplay of progesterone and estrogen, leading to ovulation, preparation of the uterus for a possible pregnancy, and shedding of the uterine lining in the form of menstruation (if conception does not happen).

All these changes require maintenance of an intricate balance between the hormones. Various factors may disrupt this hormonal balance, resulting in period symptoms but no period. The article describes various causes of period symptoms, but no bleeding, and when to consult a doctor.

Why am I having period symptoms but no period?

It is common to have premenstrual symptoms (PMS); approximately 90 percent of females experience them. Some of the common PMS symptoms are:

  • Abdominal cramps
  • Abdominal bloating
  • Fatigue
  • Headaches
  • Tenderness and heaviness of breasts
  • Mood swings
  • Difficulty sleeping

Therefore, if you are experiencing these symptoms, it is normal to assume that your period is approaching. But what does it mean to have period symptoms without a period?

Some of the causes of period symptoms but no period are:

Pregnancy

If you are trying to conceive, then period symptoms but no period can indicate a possible pregnancy. Many of the early pregnancy symptoms mimic premenstrual symptoms. When a sperm fertilizes an egg, the embryo implants into the lining of the uterus. This process may cause mild cramping.

After conception, the hormonal changes of early pregnancy may cause other symptoms such as:

  • Headaches
  • Fatigue
  • Breast tenderness and heaviness
  • Mood swings
  • Nausea
  • Vomiting

Therefore, you may feel like having period symptoms without a period due to pregnancy. If you think that you are having period symptoms but no period due to a possible pregnancy, take a pregnancy test at home.

The test result may be positive as early as 10 days post-ovulation (or 4 to 5 days before the start of a period). However, to avoid false negatives, it is advised to wait until at least the first day of your missed period.

Annovulation

Annovulation or lack of ovulation may also cause period symptoms, but no bleeding. Every month, around the 14th day of a 28-day cycle, one of the ovaries releases a mature egg. After ovulation, there is a spike in the progesterone levels as the body prepares itself for an upcoming pregnancy.

If fertilization does not occur, hormone levels decrease, resulting in the onset of menstruation. Irregularities of the menstrual cycle happen whenever ovulation is disrupted. Factors that may cause anovulation and period symptoms, but no period are:

  • Stress
  • Age
  • Weight

Age

Period symptoms, but no bleeding may occur in adolescents who have just started having their periods. This is because their menstrual cycles are irregular at first as their bodies adjust to the hormonal changes of puberty during this age.

The periods in adolescents in the age group of 10 to 15 may be:

  • Very light or spotting
  • Heavy
  • Farther apart (they may have period symptoms without a period)
  • Close together

Over time, the hormones become stable, regulating the flow and timing of periods.

Why am I having period symptoms but no period? Perimenopause and menopause may cause hormonal imbalance (a drop in the levels of estrogen), making the appearance of periods unpredictable. Hence, if you are in the perimenopausal phase, your periods may be:

  • Lighter or heavier
  • Shorter or longer

Perimenopause may start in your 40s and last till you stop having periods completely or until menopause.

Rigorous exercise

Rigorous workout or training is also a common cause of having period symptoms with a period. It commonly occurs in female athletes. Studies demonstrate that being involved in competitive sports increases the risk of menstrual irregularities up to three times.

Excessive exercise can lead to “female athlete triad’ a condition characterized by amenorrhea (no period), osteoporosis, and disordered eating patterns. Without proper management, this condition may cause weak bones, infertility, and heart problems.

Low Body Weight

Low body weight may cause hormonal disturbances, leading to anovulation and period symptoms without a period. Apart from amenorrhea (a missed period), some other symptoms include:

  • Slight spotting instead of regular menstrual bleeding
  • Headaches
  • Hair loss
  • Nipple discharge (milky white)
  • Acne

Polycystic Ovary Syndrome (PCOS)

Thyroid hormones regulate many hormonal and metabolic functions, including maintaining heart rate, body temperature, and the menstrual cycle. Thyroid conditions are characterized by the release of either too little or too much thyroid hormone in the body. Some of the symptoms are:

  • Spotting or having light bleeding during a period
  • Amenorrhea or no period
  • Heavy periods
  • Fatigue
  • Weight loss or gain
  • Restlessness
  • Problems during pregnancy
  • Infertility

Some of the causes of thyroid conditions include:

  • Stress
  • Nutritional deficiencies
  • Autoimmune diseases

Thyroid conditions occur more commonly immediately after a pregnancy or during menopause.

Stress

Why am I having period symptoms but no period? Excessive stress levels may also be one of the causes of missing a period or spotting. The stress may be:

  • Physical: implies severe illness, strict dieting, or excessive exercise
  • Emotional: due to stressful life events such as undergoing a divorce, work stress, or the death of a loved one

Studies indicate that stress, both in the family and at work, is frequently associated with menstrual irregularities.

If you suspect that you are experiencing period symptoms without a period due to stress, try doing techniques to reduce stress. Some of the activities that may help you relax and lower stress levels if done regularly are:

  • Practicing meditation
  • Practicing breathing exercises

Certain Medicines

Taking certain medicines may cause period symptoms without a period. Some of these medicines may affect hormonal balance, thereby disrupting a normal menstrual cycle. For example:

  • Birth control pills/contraceptives (containing hormones)
  • Hormonal drugs for the treatment of endometriosis (leuprolide)

Hormonal contraceptives, such as the birth control pill or injections, are made of synthetic hormones to help prevent pregnancy and regulate the menstrual cycle. After starting a new type of birth control pill, your body may need time to adapt to it. This time can range from several weeks to a few months.

While your body is adjusting to the pill, it is normal to miss a period, have irregular menstrual cycles, or experience period symptoms but no bleeding. Similar symptoms may occur while you are discontinuing the use of birth control pills.

Some other medicines may disrupt your menstrual cycle, causing period symptoms without a period as their side effects:

  • Recreational drugs
  • Blood pressure medicines (for example, methyldopa)
  • Psychotropic medicines (for example, antidepressants)
  • Medicines to treat nausea (for example, metoclopramide)

When to visit a healthcare physician?

You should visit your doctor if you are experiencing symptoms characteristic of other illnesses, such as PCOS or thyroid disease, etc. in association with period symptoms but no period. 

Seek emergency medical care if you experience the following symptoms along with period symptoms without a period:

  • Severe abdominal pain
  • Foul-smelling vaginal discharge
  • Chills or fever

Period symptoms without a period? Can you be pregnant?

If you are trying to conceive, then having period symptoms without a period can be due to a possible pregnancy. If you feel that this is the case, watch for the following early pregnancy signs:

  • A missed period
  • Nausea and vomiting
  • Food cravings and aversions
  • Tenderness in the breasts
  • Spotting
  • Abdominal cramps
  • Increase in vaginal discharge
  • Mood changes
  • Fatigue
  • Increased desire to pee
  • Bloating
  • Constipation
  • Headaches
  • Metallic taste in the mouth
  • Sensitivity of smell
